Dulaglutide

Dulaglutide is a long-acting GLP-1 receptor agonist developed by Eli Lilly, built by covalently linking a DPP-4-protected GLP-1 analog to a human IgG4-Fc fragment via a small peptide linker — a design that gives it a long half-life suitable for once-weekly dosing. It is FDA-approved as Trulicity® for type 2 diabetes and for cardiovascular risk reduction. In the research market it is sold for research use only.

What is Dulaglutide and why does the Fc-fusion design matter?

Dulaglutide delivers the standard GLP-1 mechanism — glucose-dependent insulin secretion, glucagon suppression, slowed gastric emptying, appetite reduction — in a once-weekly Fc-fusion format that resists degradation. This makes it a much larger molecule — approximately 63 kDa per the US label: two identical disulfide-linked chains, each an N-terminal GLP-1 analog sequence (90% homologous to native GLP-1(7-37)) joined to the Fc portion of a modified human IgG4 heavy chain, with an elimination half-life of about 5 days — than peptide GLP-1 analogs like semaglutide, which matters for how it's synthesized and verified.

What does the clinical evidence show?

Dulaglutide has a large approved evidence base (the AWARD program), with randomized trials showing glycemic control non-inferior to liraglutide and superior to several oral agents, plus demonstrated cardiovascular benefit in T2D: in the REWIND trial (Lancet 2019; 9,901 participants, median follow-up 5.4 years), dulaglutide reduced major adverse cardiovascular events versus placebo — 12.0% vs 13.4%; hazard ratio 0.88 (95% CI 0.79–0.99; p=0.026) — in a population spanning both established cardiovascular disease and risk factors only, which is why Trulicity's approved cardiovascular indication covers adults with "established cardiovascular disease or multiple cardiovascular risk factors". It is approved primarily for diabetes rather than weight management, though modest weight loss occurs.

How does dulaglutide compare with semaglutide, tirzepatide, and retatrutide?

Dulaglutide is the weekly Fc-fusion GLP-1 for type 2 diabetes — it is not approved for weight management, where the newer weekly agents dominate. What are the documented side effects of Dulaglutide?

The most common effects are gastrointestinal; documented serious risks include pancreatitis and a boxed warning for thyroid C-cell tumors (contraindicated in personal/family history of medullary thyroid carcinoma or MEN2). A rat carcinogenicity study showed dose-related thyroid C-cell tumors of unknown human relevance.
